Our duties
The Wellness Miami is required by the Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy of your Protected Health Information (PHI), provide you with this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.
How we use your information
We use your PHI to provide treatment, obtain payment for that treatment, and operate our clinic (for example, quality review and training). We do not sell PHI and we do not use it for marketing without your written authorisation.
Your rights
You have the right to inspect and copy your PHI, request a correction of inaccurate information, request restrictions on certain uses or disclosures, request confidential communications, and receive an accounting of disclosures we have made.
How to exercise your rights
To exercise any right above or to file a privacy complaint, contact our privacy officer at hello@thewellnessmiami.com. You will not be retaliated against for filing a complaint. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
